چکیده

Mouthpiece non invasive mechanical ventilation is more frequently used in recent years because mouthpiece ventilation modes are being introduced to available portable ventilators. Often, a patient affected by neuromuscular disease needs noninvasive mechanical ventilation. Sometimes the patient does not accept the treatment, due to excessive air leakage, claustrophobia, anxiety and congiuntivitis. The mouthpiece makes less interference with speech, better appearance, less negative impact on the patient, no risk of skin breakdown and absence of claustrophobia. The mpv has been used to treat various neuromuscular diseases such as amyotrophic lateral sclerosis, Duchenne dystrophy, Steinert dystrophy; data in literature documenting that the mouthpiece ventilation resulted an effective, preferable and comfortable alternative.

Evaluation of ventilators for mouthpiece ventilation in neuromuscular disease.

BACKGROUND Daytime mouthpiece ventilation is a useful adjunct to nocturnal noninvasive ventilation (NIV) in patients with neuromuscular disease. The aims of the study were to analyze the practice of mouthpiece ventilation and to evaluate the performance of ventilators for mouthpiece ventilation. METHODS Practice of mouthpiece ventilation was assessed by a questionnaire, and the performance of...

Daytime mouthpiece ventilation plus nighttime noninvasive ventilation improves quality of life in patients with neuromuscular disease.

Respiratory failure due to respiratory muscle dysfunction is a common problem in neuromuscular diseases,1 and respiratory muscle weakness can be reversible, relapsing, or progressive.2 In amyotrophic lateral sclerosis (ALS), the respiratory muscle dysfunction is not reversible, and respiratory failure is fatal, usually within 5 years.3 One method of measuring the decline of respiratory function...

Open-mouthpiece ventilation versus nasal mask ventilation in subjects with COPD exacerbation and mild to moderate acidosis: a randomized trial.

BACKGROUND Open mouthpiece ventilation is efficacious in patients with neuromuscular disease. We used this ventilation technique in patients with exacerbations of COPD with mild to moderate acidosis. METHODS The study was performed in 2 respiratory monitoring care units. Fifty subjects with exacerbations of COPD, breathing frequency > 25, PaCO2 > 45, and pH between 7.25 and 7.30, as well as K...

Maintaining an "oral tradition": specific equipment requirements for mouthpiece ventilation instead of tracheostomy for neuromuscular disease.

Boitano and Benditt1 have applied the scientific method to provide further understanding of an aspect of airway management that is practiced by very few centers— one that is in great need of critical investigation and dissemination. It has been demonstrated that the improvement in nocturnal ventilation provided by noninvasive means carries over into the daytime hours and improves diurnal gas ex...

Home monitoring of daytime mouthpiece ventilation effectiveness in patients with neuromuscular disease.

Mouthpiece ventilation (MPV) allows patients with neuromuscular disease to receive daytime support from a portable ventilator, which they can disconnect at will, for example, for speaking, eating, swallowing, and coughing. However, MPV carries a risk of underventilation. Our purpose here was to evaluate the effectiveness of daytime MPV under real-life conditions. Eight wheelchair-bound patients...
